According to WHO’s health definition, we developed a self-reported Chinese Health status assessment instrument which derived from Bio-psycho-social model. We developed the instrument through 3 steps: experts’ interview, pretest, official Test. Then, its reliability and validity were tested. The results showed that the assessment instrument has good psychometric quality. The final instrument consists of 75 items (there are 72 common items between male and female, 1 item designed especially for female, and 2 items for male). 
The structure of the final instrument is shown in fig 1.
